LUTEAL PHASE STIMULATION AFTER FOLLICULAR PHASE STIMULATION DURING THE SAME MENSTRUAL CYCLE (DUOSTIM) IN A REDUCED OVARIAN RESERVE POPULATION: WHEN IS IT COST-EFFECTIVE?

چکیده
To compare the number of eggs, mature eggs and blastocysts obtained during stimulation in follicular phase (FP) versus stiulation luteal (LP) same menstrual cycle (DUOSTIM) patients with decreased ovarian reserve according to count visible antral follicles (AF) at time first oocyte retrieval. It was a Prospective paired observational study. From January 2020 March 2021, forty-nine women who would be submitted vitro fertilization embryo freezing. inclusion criterion AF less than 7. All underwent two cycles stimulation, one FP other LP. Both stimulations were performed gonadotropin-releasing hormone antagonist protocol using 300 IU recombinant FSH 75 LH. (FPS) has started on day 2 LP (LPS), 5 days after During retrieval, counted (2 9 mm). These not aspirated. FPS LPS compared mean blastocysts. In second analysis, separated into groups: Group 1: egg retrieval this moment. 2: count, collection, equal or greater obtained. The parameters analyzed by comparing for each group. FPS, it required 11.01 3.96 2.67 0.83 LPS, 13.53 necessary 5.10 3.80 1.35 results statistically significant. Analyzing group 1 separately, (5.70), (4.48) (1.57) (3.39, 2.13 0,64, respectively), all also significantly higher (12.87 vs 11.04). 2, 4.46 3.15 1.12 4.08 2.81 0.88 (not significant), Greater (14.07 11.02, p <0.05). low reserve, leads embryos previous when collection is exceeds When obtained, does increase blastocysts, hand, requires more and, therefore, cost. case, cost-effective.

Endogenous thrombin potential is higher during the luteal phase than during the follicular phase of a normal menstrual cycle.
STUDY QUESTION Do thrombin generation and haemostatic parameters differ during the two phases of the menstrual cycle? SUMMARY ANSWER Total thrombin concentration is higher during the luteal phase compared with the follicular phase of the menstrual cycle. WHAT IS KNOWN ALREADY The coagulation cascade is affected by many variables, such as fluctuations in the levels of sex hormones. The studi...

background: the luteal phase defect is a common event following the ovarian stimulation. the aim of the present study was to evaluate the use of human chorionic gonadotropine (hcg) and progesterone hormones to improve the luteal phase defect. materials and methods: 60 mice were superovulated routinely with human menopausal gonadotropin (hmg) (7.5u) and hcg (10u). the mice were mated and divided...
متن کاملLuteal phase ovarian stimulation following oocyte retrieval: is it helpful for poor responders?
BACKGROUND Poor ovarian response and retrieval of no oocytes following ovarian stimulation for in vitro fertilization (IVF) is a challenging problem for both the patient and the clinician. FINDINGS Recent evidence indicates that folliculogenesis occurs in a wave-like fashion indicating that there are multiple follicular recruitment waves in the same menstrual cycle. This relatively new scient...
متن کاملFollicular and luteal phase characteristics following early cessation of gonadotrophin-releasing hormone agonist during ovarian stimulation for in-vitro fertilization.
Gonadotrophin-releasing hormone agonists (GnRHa) are widely used in in-vitro fertilization (IVF) for the prevention of a premature rise in luteinizing hormone (LH) concentrations. However, the administration of GnRHa during the follicular phase may also impair subsequent luteal function due to retarded recovery of pituitary gonadotrophin secretion. Therefore, luteal supplementation is generally...
متن کاملHormonal manipulations in the luteal phase to coordinate subsequent antral follicle growth during ovarian stimulation.
During the early follicular phase in the menstrual cycle, antral follicle sizes are often markedly heterogeneous. These follicular size discrepancies may, at least in part, result from the early exposure of FSH-sensitive follicles to gradient FSH concentrations during the preceding luteal phase. In addition, they potentially affect the results of ovarian stimulation. Indeed, pre-existing follic...
